Thank you for returning to the Medem Personal Health Record.

Intuit Health, a leader in patient-physician communication, is providing you a new way to secure your personal health information.

In 2010, Intuit Health acquired the Medem and Medfusion businesses.  Physicians who used the older Medem system have already converted to Intuit Health, and we're now converting your personal health record to our new system.

The new Intuit Health Personal Health Record (PHR) will provide you with more features and functionality, including the ability to directly integrate your PHR into your patient portal account.

And rest assured that your current PHR information is still available and secure.

If you would like to convert your Personal Health Record to the new format, please click the Convert your PHR button below.

Within 5 business days of providing the necessary identifying information, you will receive an email at the address you provide us during the conversion process.  The email will provide instructions on how to securely access your converted PHR.
